Please bring your current insurance card with you to ensure we have the most recent insurance information on file. Along with insurance, we offer the following options for payment: cash, check, Visa, MasterCard and Discover. We do request that your co-payment be paid in full at the time of your appointment.
As a courtesy to our patients with dental benefits, we will submit your claim to your insurance company. Any portion that is not expected to be covered by these benefits is the responsibility of the patient. This may include deductibles and co-payments. Please understand that this is only an estimate, not a guarantee of payment, and is based on information available to us from your insurance company.
Minor children — The person who accompanies a minor child to the appointment is considered the responsible party. That person, therefore, is responsible for any payment due at the time service is rendered. Marital status is not a consideration under any circumstance. Decreed custody or lack thereof does not alter financial responsibility.
